Outcomes of self-expandable metal stent as bridge to surgery versus emergency surgery for left-sided obstructing colon cancer: A retrospective cohort study

چکیده
BackgroundLong-term outcomes of self-expandable metal stents (SEMSs) as bridges to surgery versus emergency in the treatment left-sided obstructing colon cancer remain unclear.MethodsUsing a nationwide inpatient database Japan, we performed one-to-one propensity score matching compare overall survival, stoma requirement, postoperative complications, and length stay between SEMS groups.ResultsCompared with group, group showed worse survival (hazard ratio, 1.80; 95% confidence interval, 1.07–3.01), higher incidence ileus (8% vs. 4%, P = 0.010), longer (14 12 days, < 0.001), lower requirement (10% 29%, 0.001).ConclusionsSEMSs are associated significantly poorer ileus, stay, than is surgery.

Self-Expandable Metal Stent as a Bridge to Surgery for Obstructing Colonic Cancer: Still a Hot Topic for Debate
Colorectal cancer (CRC) is the second most common tumor in women and the third in men and accounts for 10% of all types of tumor worldwide.1 The incidence of malignant large-bowel obstruction (MLBO) in colonic cancer patients ranges from 8 to 14% and it is usually associated with a poor prognosis.2--3 Over the last decades many retrospective studies, randomized controlled trials (RCTs) and also...
متن کاملColonic Stent as Bridge to Surgery in Patients with Obstructive Left-Sided Colon Cancer
OBJECTIVE We assessed the optimal time interval between endoscopic stenting and subsequent surgery in patients with obstructive left-sided colon cancer. METHODS We reviewed the medical records of patients who underwent endoscopic colonic stenting for obstructive left-sided colon cancer between January 2009 and January 2012. Patients who had successful endoscopic intervention as a bridge to su...
متن کاملColonic stenting as bridge to surgery versus emergency surgery for management of acute left-sided malignant colonic obstruction: a multicenter randomized trial (Stent-in 2 study)
BACKGROUND Acute left-sided colonic obstruction is most often caused by malignancy and the surgical treatment is associated with a high mortality and morbidity rate. Moreover, these operated patients end up with a temporary or permanent stoma. Initial insertion of an enteral stent to decompress the obstructed colon, allowing for surgery to be performed electively, is gaining popularity. In unco...

متن کاملEndoscopic Stenting as Bridge to Surgery versus Emergency Resection for Left-Sided Malignant Colorectal Obstruction: An Updated Meta-Analysis
INTRODUCTION Emergency resection represents the traditional treatment for left-sided malignant obstruction. However, the placement of self-expanding metallic stents and delayed surgery has been proposed as an alternative approach. The aim of the current meta-analysis was to review the available evidence, with particular interest for the short-term outcomes, including a recent multicentre RCT. ...
